Information on HIPAA and
Psychiatric and Psychological Services
We at Psychiatric and Psychological Services
operate our mental health practice in full
compliance with regulations required by the Health
Insurance Portability and Accountability
Act of 1996 (also known as HIPAA). Here you
will find our "Notice
of Privacy Practices" document
and our "Client's
Rights" statement
for your review. HIPAA requires that we notify
you of your rights to privacy through our
web site, as well as through the required
forms you can read at our office. You will
also find printed versions of these documents
when you come to our office.
All of the staff at Psychiatric and Psychological
Services has received training in HIPAA privacy
requirements. Our Privacy Officer is Dave
Zachau. Mr. Zachau will gladly answer any
questions you may have related to HIPAA.
Mr. Zachau will also take any complaints
you may have, or suggestions you would like
to make to help us serve you better.
